I am looking for a competent, professional and care focussed support coordinator to oversee and deliver high quality care and support within their Sandwell and Dudley based supported living/community service team for adults with learning disabilities.
You will join a highly respected, national charity organisation who are focussed on the delivery of high quality care and the promotion of choice and independence amongst their clients.
You will work as part of the senior team in Sandwell and Dudley and play a key role in a well-coordinated service provision. You will lead, coordinator and supervise a team of support workers and promote positive outcomes for the people you support. In addition, you will deliver direct support to the people in your care and actively promote choice and independence.
A great communicator, you will liaise effectively with external agencies and work closely with families and carers.
Risk assessments, care and support planning, care reviews and supervisions will all form part of your day to day duties.
As a warm, honest and respectful person, you will demonstrate these qualities in your day to day responsibilities as support coordinator. You will have excellent organisation skills and be able demonstrate proven shift and team leading abilities.
To be considered for the role of support coordinator, you will hold the QCF level 3 or 4 in health and social care and be able to demonstrate a proven background supporting people with learning disabilities to include autism and autism spectrum disorders. You will demonstrate shift and team leading skills and be flexible to work varied hours to meet the needs of the service. This may include weekend or evening working.
Your knowledge of CQC, DoLs and Safeguarding will be extensive and you will utilise this in your day to day responsibilities.
This exciting opportunity is offering a generous salary up to £19000 per annum as well as other exciting benefits. You will receive 25 days annual leave plus bank holidays, an additional payment for taking part in the on call rota, continued training and development and other discretionary benefits.
