Job Description: Assessor
Responsible To: Business Manager
Short Description: The Assessor is responsible for completing assessments for new Service Users who are vulnerable adults suffering from Learning Disabilities, Mental Health, Challenging Behaviours and those recovering from drug and alcohol abuse.

Specific Duties: The Assessor will be specifically responsible for:
1 Building key relationships with Social Workers, Care Managers, Discharge Liaison Officers and Placement Officers
2 Undertaking full assessments of potential Service Users
3 Develop and write care packages
4 Producing care plans including support plans and risk assessments
5 Producing costing for service to be provided
6 Working closely with Care Management to confirm funding
7 Liaise with stakeholders and internal operations to facilitate move in date
8 Follow up Service User Reviews and work in the best interest of the Service Useer
9 Provide performance data and Service User satisfaction
10 Undertaking re-assessments from time to time
11 Liaising with MDT's
12 Maintaining accurate records and preparing reports
Qualifications: The following qualification, skills and experience are required for this role:
1 A minimum of 2 years previous experience as an Learning Disabilities or Mental Health Assessor
2 CQF Level 5 in Health and Social Care (or NVQ Level 4)
3 Degree in Health and Social Care preferred
4 Educated to A level
5 Some background in sales to help build the business and promote referrals
6 Good written and spoken communication skills
7 Confident in approach and empathetic
8 Competence in the use of Microsoft Office and Internet applications
9 Strong administration skills
10 Full driving licence and use of own car for business use
